Job Summary
A healthcare company is in need of a Remote Telephonic RN Case Manager.
Must be able to:
- Provide patient education to assist with self-management
- Identify gaps or barriers in treatment plans
- Make outbound calls to assess members' current health status
Applicants must meet the following qualifications:
- 3+ years of clinical experience (any setting)
- Active, unrestricted RN license
- Computer proficiency utilizing MS Office (Word, Excel, PowerPoint and Outlook)
