Job Summary

A managed care organization is searching for a person to fill their position for a Remote Transition Care Manager.

Candidates will be responsible for the following:

  • Evaluating and coordinating and implementing discharge needs
  • Identifying and referring requests to the Medical Director

Must meet the following requirements for consideration:

  • Minimum of 3 years nursing experience, in related clinical setting
  • 3 to 5 years experience preventative multidisciplinary discharge planning
  • Registered Nurse or Social Worker
