Job Summary
A leading healthcare company needs applicants for an opening for a Remote UAS Field Assessment Registered Nurse in Bronx.
Core Responsibilities Include:
- Assessing, planning and implementing care strategies for patients
- Identifying and initiating referrals for social service programs
- Managing the care plan throughout the continuum of care as a single point of contact
Must meet the following requirements for consideration:
- Ability to travel in assigned region to visit Medicaid members
- Associate's degree or higher
- Current, unrestricted RN license for the state of New York
- 4+ years of experience working within the community health setting in a health care role
- Experience in long-term care, home health, hospice, public health or assisted living
- Proficiency with MS Word, Excel and Outlook
