Job Summary

A healthcare company has an open position for a Remote UAS Field Assessment Registered Nurse in Brooklyn.

Core Responsibilities Include:

  • Assessing, planning, and implementing care strategies
  • Identifying and initiating referrals for social service programs
  • Managing the care plan throughout the continuum of care

Qualifications Include:

  • Ability to perform the NYS UAS Assessment in the member’s home
  • Current, unrestricted RN license for the state of New York
  • Associate’s degree or higher
  • 4+ years of experience working within the community health setting in a health care role
  • Experience in long-term care, home health, hospice, public health, or similar
  • Proficiency with MS Word, Excel and Outlook
