Job Summary
A healthcare company has an open position for a Remote UAS Field Assessment Registered Nurse in Monroe County.
Core Responsibilities of this position include:
- Visiting MLTC and Managed Medicaid members in need of long term care
- Conducting face-to-face UAS assessments
- Supporting members with medication reviews and connections to services
Applicants must meet the following qualifications:
- Ability to travel throughout Monroe County/Rochester and surrounding boroughs
- Current, unrestricted RN license for the state of New York
- 4+ years clinical experience
- Previous home care, hospice or field based case management experience
- New York state issued ID or ability to obtain one prior to hire
- Computer/typing proficiency; experience with email, internet research, and calendars
