Job Summary
A well-known healthcare company has a current position open for a Remote Youth Health and Social Services Coordinator in the Pharr Area.
Candidates will be responsible for the following:
- Managing the care plan throughout the continuum of care as a single point of contact
- Assessing, planning, and implementing care strategies that are individualized by patient
- Making outbound calls and receive inbound calls to assess members' current health status
Applicants must meet the following qualifications:
- The ability to travel in assigned region to visit Medicaid members in their homes and/or other settings
- 4 years of experience working within the community health setting in a health care role
- Active RN license in the state of Texas or the ability to obtain upon hire
- Experience in long-term care, home health, hospice, public health or assisted living
- Pediatric experience
