Renal Social Worker

Job Summary

A company is looking for a Renal Social Work Case Manager to provide social work support within their Chronic Kidney Disease Program.

Key Responsibilities

• Conduct telephonic and video visits with patients and caregivers to establish trust and build relationships
• Create personalized care plans and assist patients in navigating social determinants of health and community resources
• Collaborate with an interdisciplinary team to ensure cohesive, patient-centered care for high-needs patients

Required Qualifications

• Master's degree in social work and an active license with willingness to gain additional state licenses
• Minimum of five (5) years of expertise in the medical field
• Minimum of three (3) years of experience in renal care
• LCSW or LMSW or equivalent clinical license in social work preferred
• Bilingual in English and Creole, Spanish, or Vietnamese is desired
