Job Summary

Supporting the care navigation team, the per diem Resource Coordinator - Acute Care Navigation will communicate with post-acute providers, assist in discharge planning, and schedule follow-up appointments for high-risk patients in a remote role based in Ohio.

Key responsibilities:

• Collaborate with RN Acute Care Navigators and Social Workers to facilitate patient care transitions
• Communicate with post-acute providers to provide discharge planning and clinical updates
• Schedule and reschedule follow-up appointments for high-risk patients as needed

Required qualifications:

• High School Diploma
• 2-3 years of clerical experience in a medical setting
• Proficiency in Microsoft Office and electronic medical records
• Working knowledge of medical terminology
• Ability to work independently in a fast-paced environment
