Job Summary

A company is looking for a Resource Facilitator to support individuals with complex injuries by connecting them to essential resources for recovery.

Key Responsibilities

• Coordinate and maintain up-to-date information on community resources for individuals with disabilities
• Assess patient needs and create tailored plans to access appropriate services
• Guide patients and families through application processes for various support services

Required Qualifications

• Bachelor's or master's degree in social work, Human Services, Psychology, or a related field
• 2+ years of experience in case management, social services, or resource coordination
• Familiarity with local, state, and national support programs
• Knowledge of disability rights and community-based services
• Ability to work independently and as part of a team
