Job Summary
To support a progressive pharmacy-managed medication reconciliation practice model, the full-time remote Rhode Island Licensed Pharmacy Technician will accurately obtain and compile detailed home medication histories and allergy information for patients being admitted to the hospital, while effectively interacting with patients, families, and healthcare providers.
Key responsibilities
- Obtains and compiles current home medication history lists and allergy information from various sources, including patient interviews and outpatient pharmacies
- Documents detailed and verified medication lists and allergy information in the electronic medical record (EMR) with high accuracy
- Communicates discrepancies in medication histories to healthcare providers and participates in quality improvement activities
Required qualifications
- High school graduate or equivalent; completion of one to two years of college preferred
- Current unrestricted Rhode Island Pharmacy Technician licensure and National Pharmacy Technician Certification (CPhT) or one year of outpatient pharmacy technician experience
- Minimum one year of pharmacy technician experience in an outpatient pharmacy setting
- Strong customer service skills and the ability to communicate effectively with patients and healthcare staff
- Proficient in Microsoft Office Suite and comfortable using various pharmacy and system-wide applications
