Job Summary

A company is looking for a Population Health & Wellness Practice Nurse.

Key Responsibilities:

• Review health plan data to develop clinical findings and future spending projections
• Coordinate clinical care with health insurance carriers and assist in health risk management planning
• Support members in accessing healthcare resources and appealing insurance denials

Required Qualifications:

• Must be a Registered Nurse
• Certified Case Manager is a plus
• At least 5 years of clinical experience, preferably in insurance or Managed Care
• Proficiency in Microsoft Office applications
• Experience with data analytics platforms to identify care gaps
