Rhode Island RN Case Manager

Job Summary

A company is looking for an RN Case Manager to coordinate patient care across various needs and collaborate with a multidisciplinary team.

Key Responsibilities

• Conduct initial intake calls and ongoing patient outreach to review care plans and monitor progress
• Provide care coordination for complex patients and develop plans of care in partnership with the care team
• Support patients during care transitions and collaborate with a multidisciplinary team to address gaps in care

Required Qualifications

• Current RN License in good standing in Rhode Island or an unencumbered multistate RN license
• Minimum associate degree in nursing required; bachelor's degree preferred
• At least 3 years of clinical experience, with 1 year in case management in relevant settings
• Certified Case Manager (CCM) certification or commitment to obtain it when eligible
• Geriatric care experience and knowledge of Medicare and Medicaid regulations preferred
