A company is looking for an RN Cardiac Health Coach - Wellness.
Key Responsibilities
- Complete health assessments and implement care plans for members with cardiac conditions
- Provide telephonic support and coaching to improve members' health and lifestyle
- Monitor wellness plans and adjust care recommendations based on member needs
Required Qualifications
- Associate's degree with 2 - 4 years of related experience
- Registered Nurse (RN) license required
- Health Coach Certification or equivalent certification in a related field
- Experience with chronic health conditions, particularly cardiac issues
- Ability to engage and influence members in health-related decisions
