Job Summary

Providing triage and health advice by phone, the part-time RN Care Line - MN Licensed will utilize evidence-based protocols to assess patient symptoms, process prescription refill requests, and coordinate patient transfers in a remote setting.

Key responsibilities

• Deliver triage and health advice to patients, utilizing computerized protocols for symptom assessment
• Process prescription refill requests from pharmacies and patients while documenting all calls electronically
• Assist with the coordination of patient transfers and participate in transitional care calls for discharged patients

Required qualifications

• Current RN license in Minnesota, North Dakota, and/or Wisconsin, with the ability to obtain all three within three months of hire
• Minimum of 2 years of experience in telephone triage, transitional care management, or remote patient monitoring
• Experience with ambulatory computer software systems
• Availability for day/evening shifts, including weekends and holidays
