RN Care Manager

Job Summary

A company is looking for a Care Manager, RN.

Key Responsibilities

• Conduct comprehensive assessments of patients' medical, psychosocial, and functional status
• Develop patient-centered care plans and educate patients and caregivers on their disease processes
• Act as a clinical liaison, communicating patient health updates and treatment plans to providers and caregivers

Required Qualifications

• Active, unrestricted compact RN license
• 2+ years of nursing experience in various care settings
• Knowledge of pathophysiology and treatment protocols for common health diagnoses
• Proficient computer skills and ability to adapt to various technology platforms
• Ability to work independently and as part of an interdisciplinary care team
