Job Summary

A company is looking for a Registered Nurse Care Manager for a fully remote position.

Key Responsibilities:

Identifying members in need of additional assistance and enrolling them in care management services
Developing and implementing individualized care plans based on members' needs and goals
Coordinating communication among healthcare team members and relevant parties to ensure quality care

Required Qualifications:

Current RN compact license to practice in all states that are part of the compact
Experience in clinical nursing in an ambulatory care setting
Experience in care management, managed care, health promotion, or related roles
Bachelor's degree in Nursing (BSN) from an accredited institution
Case Management Certification is preferred
