RN Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop ongoing care plans and collaborate with providers to address unmet needs
• Provide resource support and education to members and their families on healthcare options

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in case management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Experience in telephonic, digital, or home outreach assessments preferred
• Availability to work in Eastern Time zone hours
