Job Summary

A company is looking for an RN Care Manager - Hybrid/Remote.

Key Responsibilities

Manage care for patients with multiple chronic diseases, focusing on disease education and health coordination
Identify and mitigate social influencers of health while collaborating with provider offices for integrated care
Ensure improved health outcomes through effective care plan coordination and service delivery

Required Qualifications

Associates Degree required; BSN preferred
5 years of RN experience in care management settings
Previous electronic medical record (EMR) experience required
Certification in Case Management (CCM) preferred
Strong knowledge of community resources and insurance plans
