RN Care Manager - Indiana

Job Summary

A company is looking for an RN Care Manager - MBB - Haitian Creole RN.

Key Responsibilities:

• Collaborate with interdisciplinary care team to improve quality of care and meet individual needs
• Engage members to develop person-centered care plans and address health needs
• Facilitate coordination and communication with providers and community resources

Qualifications:

• Nursing degree or Bachelor's degree in a health care field
• Minimum of three years experience in nursing, social work, or health care profession
• Three years of Medicaid and/or Medicare managed care experience preferred
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor
• Case Management Certification highly preferred
