RN Care Navigator

Job Summary

A company is looking for a RN Care Navigator (100% Remote for Eastern or Central time zones).

Key Responsibilities

• Manage in and outbound calls while delivering exceptional service to members
• Educate members on kidney health, related co-morbid conditions, and renal replacement therapy options
• Coordinate care as a liaison among patients, their support networks, and healthcare providers

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
