RN Care Navigator

Job Summary

Managing a caseload of kidney health patients, the full-time RN Care Navigator will coordinate care, conduct health screenings, and educate members on renal replacement therapy options while working remotely.

Key responsibilities

• Handle in and outbound calls to deliver exceptional service to members
• Educate members on kidney health and related co-morbid conditions, optimizing renal replacement therapy
• Collaborate with healthcare providers to ensure effective care coordination and resolve patient concerns

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; or 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
