RN Care Navigator

Job Summary

Managing a caseload of patients, the full-time RN Care Navigator will coordinate kidney health management, facilitate care delivery, and promote high-quality outcomes while working remotely.

Key responsibilities

• Handle inbound and outbound calls, providing exceptional service and education on kidney health and treatment options
• Engage members in HealthMap's Kidney Health Program and follow up based on complexity and policy guidelines
• Collaborate with healthcare providers and community resources to coordinate care and resolve patient concerns

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
