Job Summary

A company is looking for an RN Care Navigator in Pennsylvania.

Key Responsibilities:

• Handle in and outbound calls providing excellent service to members
• Educate members on kidney health and related conditions, optimizing renal replacement therapy
• Collaborate with healthcare personnel to ensure positive patient outcomes and care coordination

Required Qualifications:

• Pennsylvania RN license
• Three years of case management experience preferred
• Experience in a dialysis or transplant center preferred
• Experience with Medicare and Medicaid preferred
