Job Summary
A healthcare organization is seeking a Registered Nurse Case Manager for a remote contract position.
Key Responsibilities:
- Lead a multidisciplinary team to provide holistic care management to members telephonically and digitally
- Assess, develop, implement, monitor, and evaluate care plans to optimize member health
- Coordinate care delivery, advocate for members, and provide education on health management
Required Qualifications:
- Nursing Diploma or Associate's degree in nursing; Bachelor's degree preferred
- 3 years of clinical nursing experience, 1 year of case management experience in a managed care setting
- Current, active, and unrestricted Registered Nurse license (Compact License)
- Certification in Case Management (CCM) required or to be obtained within 18 months of hire
- Certification in Chronic Care Professional (CCP) preferred
