Key Responsibilities

Engage with patients to assess and prioritize their clinical support needs

Required Qualifications

Bachelor's Degree in Nursing
5 years of experience as an RN or RN Care Manager
Post Acute Care and Transitions of Care experience strongly preferred
Unencumbered RN license; compact nursing license preferred
Accredited Case Manager (ACM) preferred
