Job Summary

A company is looking for a Clinical Guide: Case Manager, Gold Care.

Key Responsibilities

• Develop long-term patient relationships and conduct holistic assessments
• Create and evaluate care plans in collaboration with the interdisciplinary team and caregivers
• Support chronic disease management and ensure smooth transitions of care

Required Qualifications

• An unrestricted compact RN license
• RN Complex Case Management experience
• A minimum of 5 years of direct clinical experience in acute care
• Strong knowledge of interdisciplinary teamwork
• Comfort with participating in interdisciplinary discussions
