RN Case Manager

Job Summary

A company is looking for a Case Management Nurse to join their Case Management team.

Key Responsibilities

• Assist in the coordination of care across various settings including inpatient, outpatient, and home care
• Develop care plans for members facing health challenges and communicate with them to provide education on health conditions
• Proactively collaborate with hospital case managers for discharge planning and ensure compliance with applicable laws and regulations

Required Qualifications

• Active, unrestricted RN licensure from the United States or a compact multistate unrestricted RN license
• Ability to obtain additional state licenses as needed
• 2+ years of clinical experience in payer, hospital, outpatient, or community-based care management
• 1+ years of experience in Care Coordination and Navigation
• CCM Certification and BSN are considered a bonus
