Job Summary

A company is looking for an RN Case Manager to provide member-centered care and coordinate services across various healthcare settings.

Key Responsibilities

• Assess individual care goals and coordinate medical and supportive services for members
• Facilitate smooth transitions for members between different care settings
• Connect members with community resources to support their needs and objectives

Required Qualifications

• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical/acute care experience
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred, or commitment to obtain within two years of hire
