RN Case Manager

Job Summary

A company is looking for a Case Manager to act as a liaison between patients and healthcare providers.

Key Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate patient care options
• Serve as the primary contact for patients, primary care physicians, and other healthcare providers
• Ensure effective communication and collaboration within the healthcare community

Required Qualifications

• Active RN License required
• Associate's Degree in Nursing
• Minimum three years of clinical care experience
• Ability to obtain CCM certification within 18 months of hire date
• Bilingual preferred
