RN Case Manager

Job Summary

A company is looking for a Case Management Nurse.

Key Responsibilities

• Coordinate patient care and progression by reviewing cases and developing care plans
• Conduct comprehensive assessments and create individualized transition plans with interdisciplinary teams
• Facilitate safe transitions of care and ensure effective communication of care plans to patients and caregivers

Required Qualifications

• RN compact unrestricted license required; BSN preferred
• 3-5+ years of case management, care coordination, or acute care nursing experience
• Strong clinical judgment and understanding of care transitions
• Experience with utilization management, discharge planning, or population health preferred
