RN Case Manager

Job Summary

Providing member-centered care remotely, the full-time RN Case Manager will assess individual care goals, coordinate medical and supportive services, and connect members with community resources to facilitate smooth transitions between care settings.

Key responsibilities

• Assess individual care goals and develop personalized care plans for members
• Coordinate medical and supportive services, ensuring smooth transitions between various care settings
• Connect members with community resources to support their healthcare needs and objectives

Required qualifications

• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical/acute care experience
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred, or ability and commitment to obtain within two years of hire
• Experience working with vulnerable and complex populations in a clinical, home care, or telephonic environment
