RN Case Manager

Job Summary

Providing member-centered care remotely, the full-time RN Case Manager III will assess individual care goals, coordinate medical and supportive services, and connect members with community resources to ensure smooth transitions across various care settings.

Key responsibilities

• Assess individual care goals and develop personalized care plans
• Coordinate medical and supportive services for members across multiple care settings
• Connect members with community resources to support their health needs and objectives

Required qualifications

• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical/acute care experience
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred, or ability to obtain within two years of hire
