RN Case Manager
Job Summary
Providing telephonic, short-term interdisciplinary care management, the full-time RN Case Manager will engage with high-risk patients post-discharge to coordinate care, develop care plans, and ensure access to necessary resources in a remote setting.
Key responsibilities
- Engage with newly discharged patients to assess needs and develop tailored care plans
- Collaborate with healthcare providers to ensure coordinated post-discharge care and smooth transitions
- Educate patients on chronic conditions and medication adherence while addressing potential barriers to care
Required qualifications
- Unrestricted Compact RN license obtained in the United States
- 3-4 years of RN Care Management or telephonic case management experience
- Availability to work Monday-Friday from 8:30 AM to 5:00 PM MST/PST
- Experience in health insurance, particularly Medicare Advantage
- Clinical analytical thinking to address individual patient needs
