Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities:

• Assess, plan, implement, coordinate, monitor, and evaluate options and services for individuals' health needs
• Serve as a liaison to facilitate collaboration among patients, families, physicians, and other providers
• Ensure compliance with regulations, policies, and standards while practicing within the scope of licensure

Required Qualifications:

• Bachelor's degree in Nursing preferred
• Registered Nurse (RN) with current active state license in good standing
• Case management certification such as CCM, ACM, or RN-BC required
• Minimum four years' clinical practice experience in relevant areas
• Ability to work independently, interpret complex documentation, and make sound judgments based on guidelines
