Job Summary

A company is looking for an RN Case Manager-Certified Diabetes Care + Education Specialist (CDCES) for a remote position in the US.

Key Responsibilities:

Provide care coordination and case management to beneficiaries
Develop and implement diabetes educational sessions for staff, beneficiaries, and families
Collaborate with the multidisciplinary care team to ensure beneficiary needs are met

Required Qualifications/Education:

Current, unrestricted RN License in a state or territory of the United States
Associate's Degree in Nursing
3+ years of clinical experience, with 1-3 years of case management or home care experience
1+ years of experience as a Certified Diabetes Care and Education Specialist (CDCES)
Current Certified Diabetes Care and Education Specialist (CDCES) certification
