Job Summary

A company is looking for a RN Case Manager - Compact License.

Key Responsibilities

• Lead a multidisciplinary team to provide holistic care to meet member needs through telephonic and digital communication
• Utilize the case management process to develop, implement, and evaluate individualized care plans
• Coordinate resources and advocate for members to improve overall health outcomes

Required Qualifications

• Nursing Diploma or Associate's degree in nursing required
• Bachelor's degree in nursing strongly preferred
• 3 years of clinical nursing experience in various healthcare settings required
• 1 year of case management experience in a managed care setting strongly preferred
• Current, active, and unrestricted Registered Nurse license (Compact License) required
