Job Summary

A company is looking for an RN Case Manager - Compact License Required - Remote.

Key Responsibilities

• Make outbound and receive inbound calls to assess members' health status and care gaps
• Complete assessments for clinical and social needs and manage the plan of care
• Collaborate with Primary Care Physicians and other departments regarding member needs

Required Qualifications

• Current, unrestricted RN compact license
• 3+ years of experience in a hospital, acute care, or direct care setting
• Basic level of experience with Microsoft Office (Word, Excel, Outlook)
• Designated workspace and access to install high-speed internet via hardwire in home
