Job Summary

A company is looking for a RN Case Manager - Compact License.

Key Responsibilities

Lead coordination of a multidisciplinary team to provide holistic care to members
Utilize the case management process to assess, develop, implement, and evaluate care plans
Engage members and caregivers to create individualized plans of care and arrange necessary resources

Required Qualifications

Nursing Diploma or Associate's degree in nursing required
Bachelor's degree in nursing strongly preferred
3 years of clinical nursing experience in a clinical or community setting required
1 year of case management experience in a managed care setting strongly preferred
Current, active, and unrestricted Registered Nurse license (Compact License) required
