Job Summary

A company is looking for a Transitions of Care RN Case Manager.

Key Responsibilities

• Engage with newly discharged patients to assess their needs and develop care plans
• Coordinate care with healthcare providers and community resources to ensure comprehensive support
• Educate patients on chronic conditions and facilitate medication adherence

Required Qualifications

• Unrestricted Compact RN license in the United States
• 3-4 years of RN Care Management or telephonic case management experience
• Availability to work Monday-Friday during specified hours
• Experience in health insurance, particularly Medicare Advantage, is preferred
• Clinical analytical thinking skills for individualized patient care
