RN Case Manager - Compact License

Job Summary

To support health optimization, the remote RN Case Manager - Compact License will manage member assessments, evaluate clinical and social needs, and collaborate with healthcare providers while working full-time within a Monday to Friday schedule from 8 AM to 5 PM local time.

Key responsibilities:

• Make outbound and receive inbound calls to assess members' health status and identify gaps in care
• Complete assessments and manage plans of care, ensuring timely referrals and communication with members
• Collaborate with primary care physicians and other departments to address medication concerns and member needs

Required qualifications:

• Current, unrestricted RN compact license
• 3+ years of experience in a hospital, acute care, or direct care setting
• Basic proficiency in Microsoft Office (Word, Excel, Outlook)
• Designated workspace with high-speed internet access for remote work
