RN Case Manager - Compact License

Job Summary

To support complex case management needs, the full-time RN Case Manager - Compact License will coordinate care for members with multiple comorbidities, conducting assessments and developing action plans while working remotely for candidates residing in the Mid-West and available for CST and EST time zones.

Key responsibilities:

• Provide transition of care calls and conduct assessments to ensure members receive necessary services post-discharge
• Collaborate with interdisciplinary care teams to develop and monitor individualized care plans for members
• Advocate for members and their families, linking them to community resources and ensuring adherence to care management standards

Required qualifications:

• Current, unrestricted RN license with Compact State Licensure
• 3+ years of diverse clinical experience, preferably with acutely ill members
• Knowledge of discharge planning processes
• Proficiency in Microsoft Office applications including Outlook, Word, Teams, and Excel
• Currently residing in the Mid-West and able to work with CST and EST time zones
