RN Case Manager - Compact License

Job Summary

Working remotely, the full-time RN Case Manager - Compact License will assess members' health status, complete evaluations for clinical and social needs, and manage care plans while collaborating with healthcare providers.

Key responsibilities:

• Make outbound and receive inbound calls to assess members' health status and identify care gaps
• Complete timely evaluations of members for clinical and social needs and manage referrals as necessary
• Communicate and update care plans while collaborating with primary care physicians and other departments

Required qualifications:

• Current, unrestricted RN compact license
• 3+ years of experience in a hospital, acute care, or direct care setting
• Basic level of experience with Microsoft Office (Word, Excel, Outlook)
• Designated workspace with access to high-speed internet via hardwire
