Job Summary
RN Case Manager - Compact State Licensed, is a full-time position responsible for creating, implementing, and monitoring treatment plans to assist patients in achieving their healthcare goals, while coordinating care and assessing needs.
Key Responsibilities
- Assess member health status and coordinate care needs, including inpatient review and discharge planning
- Develop and implement case management goals with documented care plans and monitor treatment effectiveness
- Identify and coordinate services, resources, and providers to ensure timely and cost-effective care for members
Required Qualifications
- 5+ years of related work experience in clinical nursing and patient assessment
- Bachelor's degree or equivalent related work experience
- Graduate of an accredited school of nursing
- RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
- Knowledgeable in medical terminology and cost containment practices
