Job Summary

RN Case Manager - Compact State Licensed, is a full-time position responsible for creating, implementing, and monitoring treatment plans to assist patients in achieving their healthcare goals, while coordinating care and assessing needs.

Key Responsibilities

• Assess member health status and coordinate care needs, including inpatient review and discharge planning
• Develop and implement case management goals with documented care plans and monitor treatment effectiveness
• Identify and coordinate services, resources, and providers to ensure timely and cost-effective care for members

Required Qualifications

• 5+ years of related work experience in clinical nursing and patient assessment
• Bachelor's degree or equivalent related work experience
• Graduate of an accredited school of nursing
• RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
• Knowledgeable in medical terminology and cost containment practices
