RN Case Manager - Compact State Licensed

Job Summary

Managing treatment plans for patients, the full-time RN Case Manager - Compact State Licensed will assess health needs, coordinate care, and monitor progress remotely while collaborating with healthcare providers and community resources.

Key responsibilities

• Assessing members' health status and coordinating care, including inpatient reviews and discharge planning
• Developing and implementing treatment plans while identifying alternative care options in line with cost containment guidelines
• Monitoring health status and treatment effectiveness, adjusting care plans as necessary based on client needs

Required qualifications

• 5+ years of related work experience in clinical nursing and patient assessment
• Bachelor's degree or equivalent work experience in a related field
• Graduate of an accredited school of nursing
• RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
• Knowledge of medical terminology and cost containment practices in healthcare
