Job Summary
Responsible for creating, implementing, and monitoring treatment plans, the full-time RN Case Manager - Compact State Licensed will work remotely to assist patients in meeting their healthcare goals through assessment, care coordination, and collaboration with healthcare providers.
Key responsibilities
- Assess member health status and coordinate care needs, including inpatient review and discharge planning
- Develop and implement individualized treatment plans while monitoring health status and adjusting care as needed
- Identify and coordinate services and resources to promote optimal value and cost-effectiveness for clients
Required qualifications
- 5+ years of related work experience in clinical nursing and patient assessment
- Bachelor's degree or equivalent related work experience
- Graduate of an accredited school of nursing
- RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
- Knowledge of medical terminology and cost containment practices in healthcare
