Job Summary

Managing treatment plans for patients, the full-time RN Case Manager - Compact State Licensed will assess healthcare needs, coordinate care, and monitor progress remotely while ensuring compliance with healthcare best practices and cost containment guidelines.

Key responsibilities

• Assess member health status and care coordination needs, including inpatient reviews and discharge planning
• Develop and implement case management goals with documented care plans while collaborating with patients, families, and healthcare providers
• Monitor treatment plans and adjust care as necessary based on changing client needs and outcomes

Required qualifications

• 5+ years of experience in clinical nursing and patient assessment
• Bachelor's degree or equivalent work experience in a related field
• Graduate of an accredited school of nursing
• RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
• Knowledge of medical terminology and cost containment practices in healthcare
