RN Case Manager - Compact State Licensed

Job Summary

Managing treatment plans for patients, the full-time RN Case Manager - Compact State Licensed will create, implement, and monitor care strategies while working remotely and coordinating with healthcare providers to ensure effective patient outcomes.

Key responsibilities:

• Assess members' health status and coordinate care needs, including inpatient and outpatient management
• Develop and implement short- and long-term case management goals with documented care plans
• Monitor health status and adjust care plans based on changing client needs and treatment effectiveness

Required qualifications:

• 5+ years of relevant experience in clinical nursing and patient assessment
• Bachelor's degree or equivalent related work experience
• Graduate of an accredited school of nursing
• RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
• Knowledge of medical terminology and cost containment practices in healthcare
