Job Summary

A company is looking for a RN Case Manager - Optum at Home - Remote.

Key Responsibilities

• Assess health status and establish care goals for members
• Plan, implement, and evaluate care plans in collaboration with the care team
• Educate patients and families on health maintenance and coordinate community resources

Required Qualifications

• Current unrestricted RN licensure in the state of residence
• Compact multistate RN license
• 2+ years of experience as an RN
• 1+ years of Case Management experience
• Experience in assessing complex medical and social needs
