RN Case Manager - Louisiana Licensed

Job Summary

A company is looking for an RN Case Manager Expert (IKC).

Key Responsibilities

• Stabilize high-risk Integrated Kidney Care (IKC) patients through intensive care coordination
• Coordinate care among patients, families, physicians, and healthcare teams to ensure quality and cost-effective care
• Function autonomously while collaborating with the healthcare team to minimize fragmentation in care delivery

Required Qualifications, Training, and Education

• A.D.N degree from an accredited school of nursing required; B.S.N preferred
• Current Registered Nurse (RN) license in the state of practice required
• Minimum of five (5) years' experience in clinical nursing required
• Minimum of three (3) years' experience in renal nursing preferred
• Current CPR certification required
