RN Case Manager - Maternity

Job Summary

A company is looking for an RN Case Manager - Maternity/OB.

Key Responsibilities

• Make outbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and coordinate care for members
• Provide patient education and support for self-management and healthy lifestyle changes

Required Qualifications

• Active, unrestricted RN license in a compact state
• 3+ years of clinical experience in Maternity/OB
• Proficiency in MS Office and ability to navigate a Windows environment
• Access to high-speed internet and a dedicated workspace for remote work
• Ability to obtain additional licenses if needed
